A Message from the School Nurse Ms. Anderson

 

Dear Parents / Guardians of Delia Bolden School:

 

What a year we got through together with flexibility, pandemic diligence, and care for each other!

 

As we draw near to the end of this 2022-2023 school year, we want to thank you for your support in providing a safe and healthy learning environment for your child.  During this summer break, we hope you and your families explore opportunities for balanced eating, exercise and moments for fun and relaxation. Summer is also a great time to prepare for the new school year.  Please note the following as it pertains to your child:

 

Medications - Students requiring medications that are to be given by the school nurse during school hours - including over the counter medications - must have a yearly updated Request for Administration of Medication Form*on file in the nursing office at the beginning of each school year.

 

Asthma – Students with asthma must have a yearly updated Asthma Treatment Plan* on file in the nursing office at the beginning of each school year.

 

Life-Threatening Allergies - Students with life-threatening allergies must have a yearly updated  Anaphylaxis Emergency Health Care Plan* on file in the nursing office at the beginning of each school year.

 

Seizure Disorder - Students with active seizure disorder must have a yearly updated Seizure Action Plan Form.*

 

Physical Examination - although not required after enrollment, we remind you that  it is important to obtain annual medical examinations for your children.  We would appreciate it if your child’s  pediatrician would fill out the Physical Examination Form.*

 

Therefore, please reach out to your Health Care Provider (HCP) as soon as possible to schedule an appointment to have these forms completed before the start of the new school year.  *Forms can be obtained on the District website under Departments=> Nursing Services=> or by clicking the link below:

Completed forms may be submitted or emailed to the school nurse.

 

As a reminder, all medications need to be picked up by a parent/guardian before the last day of school or they will be discarded. Medications will not be given to students to bring home.  Please contact Tanisha Anderson and I will arrange to meet you at the front door with medications.
